The form asks for the moving contractor's license information, both locations, the route and loaded height, width and length. Because this is an older form still listed by the city, confirm its current submission instructions and applicability with Planning before relying on its printed conditions.
Build a route record that reviewers can use
Once the city confirms that the application is appropriate, have the mover supply a proposed street route and the shipment measurements together. Keep the origin and destination property details attached to the same record so a route change does not leave the application describing a different move.

Provide the pickup and delivery addresses, each section's loaded dimensions and any city review already completed. Ask the transporter to identify route work still needed before scheduling.
The form provides approval spaces for police, the building inspector and utility representatives. Ask which signoffs are currently required for your shipment and who will obtain each one. A blank form or a submitted request is not evidence that those reviews are complete.
